The Farm in Stouffville is an addiction and mental health treatment centre for adults 18 years of age and older.

Addiction intake at The Farm is based on each individual’s needs and the suitability of our programming. The first step of the admission process is a thorough intake assessment, this is done to determine each individual’s readiness to change and to gain a deeper understanding of our potential patients, their dependencies and their motivation to seek treatment. This step is crucial for us to develop the best individualized treatment plan possible tailored to the patient’s needs.

During the intake assessment you will be asked questions about your history with addiction and/or mental health concerns, previous treatment or counselling history, brief medical history, trauma history and what your goals are for treatment at The Farm Rehab.

After the intake assessment has been completed and our intake specialists determine that an individual’s unique needs can be supported, an intake/admission date will be scheduled.
